This is the new form for use from April 19th 2004.

South Australian forms must be obtained from Mr Brokus. Complete all parts of form and obtain signatures of employer, parent/guardian and yourself. Return to Careers Teacher who will have Principal sign form. Also have employer complete Form 15 below.

You will then be given two copies, one for yourself & your parents and one for the employer.
